Choose your PCP

An important part of your health care experience at Kaiser San Rafael includes building a relationship with your PCP. If haven't already selected one or want to change, click here to choose a PCP for yourself or for a family member online. If you are already linked with a personal physician, you can request to make a change. If you are a woman, we encourage you to select a PCP in Obstetrics/Gynecology in addition to one in Internal Medicine or Family Practice. Please have your Kaiser card available as your request can not be completed without that information.

Please note that in some locations you may also choose a nurse practitioner or physician assistant as your personal practitioner. Some physicians and health care practitioners may be unable to accept new patients because their practices are completely full. In such instances, we can assist you in making another selection.
